Middle School Immunizations


Effective July 1, 2007, all children entering the sixth grade or new entrants to Georgia schools will need:
  • 2 doses of MMR or 2 doses of Measles vaccine, 2 doses of Mumps vaccine and 1 dose of Rubella vaccine, or year of serology.
  • 2 doses of varicella (Chicken pox vaccine) or a date of history of the disease, diagnosis, or serology.
Your local health department or your pediatrician has the vaccines and the forms available. Please have these immunizations completed prior to the first day of school.

Documentation of immunizations should be made on the #3231 form. All forms should be sent to the Middle School office. We encourage all families to have this completed and sent in as soon as possible.
