Instructions for Teacher Evaluation Forms
There are three different form options:
- K3, K4 and Pre-first (rising three, four and five year old classes)
- First through Fourth (rising 1st-4th)
- Fifth through Twelfth (Forward this form for rising 5th-12th graders to both a mathematics and an English teacher.)
