Instructions for Teacher Evaluation Forms


There are three different form options:
When forwarding the evaluation form to a teacher, please provide a stamped envelope for their use, using the following address:

Please note that we do NOT receive mail at our street address.

DUE DATES:

For K3-1st, have recommendations returned by January 15
For 2nd-12th, have recommendations returned by March 15
